LPN - Clinical Coordinator

This is an Entry Level Management Position.

Health Care Plus, a certified Medicare Home Care Provider and one of the Marden Companies, is searching for a Clinical Coordinator.

The qualified candidate would work at our Health Care Plus Columbus Branch Office.

This is an entry level, full time position that offers exceptional salary and an excellent benefit package.

This position is instrumental in assisting to coordinate day to day activities of staff, ensuring patient care needs are met, and continuously working in a team environment.

Excellent verbal, written, computer and customer service skills a must. This position requires a great deal of contact with patients, staff, referral sources and the communities we serve.

Some minor travel will be required and patient visits may be required when necessary.

This position is full time day shift position, Monday through Friday, with holiday coverage if needed.

IV Certification is a plus but not required.

Requirements Include:
* 5 Years of strong clinical/care coordination experience
* 3 years of experience for the right candidate
* Electronic medical record documentation proficiency
* All other patient care skills as required of an LPN

Job responsibilities include but are not limited to

1. Processing EMR workflow in a thoughtful and thorough manner utilizing critical thinking and problem solving skills.
2. Coordinating weekend on call and clinic schedules
3. Working directly with all field staff to assist and facilitate care coordination
4. Daily contact with referral sources and physicians to obtain verbal orders, clarify orders, request pertinent medical information.
5. Assisting the company administration with projects, information gathering, payroll, credentialing and any other assistance administration may ask of this position
6. Working directly with the Administrator and Director of Nursing.
7. Function as one of the the main clinical contacts in the Director of Nursing's absence.
8. Participate in manager on duty on call rotation
